What Is Surgically Assisted Orthodontics?

While many orthodontic cases are straightforward, some require oral surgery for optimal success. Dr. Nojoumi and Dr. Namiranian work with trusted local orthodontists by extracting teeth, exposing impacted teeth, and clipping the frenulum (correcting tongue or lip tie).

Benefits of Surgically Assisted Orthodontics

  1. May save impacted teeth from extraction by exposing them and allowing the orthodontist to adjust their position
  2. May prevent impacted teeth from resorbing and damaging healthy permanent teeth
  3. Helps with situations in which the teeth have severe crowding
  4. Prepares children for later orthodontic work
  5. Corrects tongue tie and lip tie, helping young children feed and speak properly, and closes spaces between teeth

Procedures Offered

Impacted Tooth ExposureAn impacted tooth cannot normally emerge into the mouth due to its angle or position in the bone. These teeth pose a risk to their neighboring teeth, as they can resorb healthy tooth roots as they continue to move through the bone. Oral surgeons can surgically expose these teeth and attach a metal bracket for the orthodontist so that they can be pulled into their proper position with braces.

Impacted Teeth ExtractionsIf impacted teeth cannot be saved or if they are impacted wisdom teeth, sometimes they require extraction. If left untreated, impacted teeth can cause root resorption of adjacent teeth and/or develop cysts around their crowns. These teeth benefit from extraction to preserve overall oral health as well as the surrounding teeth.

Premolar Extractions- Together with your orthodontist, occasionally, the decision is made to extract premolars prior to starting braces. This decision is usually made due to severe crowding and usually includes the removal of two to four of your premolars.

Frenectomies- Frenula are small folds of tissue in the mouth. The most commonly treated oral frenula are the maxillary frenulum and the lingual frenulum. The lingual frenulum attaches the tongue to the floor of the mouth. However, in some patients, the lingual frenulum is so tight that they have difficulty eating or speaking clearly. A tight maxillary frenulum can also be problematic if it interferes with the closure of spaces orthodontically. For these frenula, our team coordinates the timing of the frenectomy with your orthodontist to easily correct this problem.

Extraction of Baby Teeth for Guided Eruption- Sometimes, baby teeth interfere with the eruption of permanent teeth. Oral surgeons collaborate with orthodontists to decide which baby teeth need to come out to help the permanent teeth come in straight.

Frequently Asked Questions About Surgically Assisted Orthodontics

Should a child have a baby tooth extracted if it is in the way of other teeth?

In most cases, the orthodontist will recommend an extraction if the tooth interferes with other teeth.

How long does it take to extract an impacted tooth?

The length of time it takes to remove the tooth depends on its condition. Depending on the circumstances, extraction may take as little as 20 minutes per impacted tooth.

How long does moving an exposed tooth to its final position take?

Moving the tooth to its permanent position may take up to a year because the orthodontist carefully controls the process.

Are premolars difficult to extract?

Premolars can be challenging because they often have crooked or hook-like roots. Our expert oral surgeons can remove them very comfortably.

Why is it important that children receive frenectomies?

Parents may feel inclined to leave a tight tongue tie or lip tie alone, but this problem can cause feeding, speech, and orthodontic problems later in life. It is best to have an evaluation when in doubt to see if the tongue tie or lip tie needs to be addressed.

I’m not a patient at the center, but the help I have received from them is beyond words. I’m here at UCDavis to help my niece (19 year old international student) with her medical and dental care needs after an electric scooter accident. I work as an administrator in medical field in another State, it has been very hard for me to get things done that she needs here. I was looking for a dental surgeon after her jaw surgery, and nobody from School Student Health and insurance company could point me to the right direction. The provider’s list provided by the insurance company is outdated. I was searching online and I found the Center. I didn’t see the Center is on the insurance list, but a feeling told me to give them a call. I’m so glad I did. Angela was the sweetest person I have encountered over the phone. Her compassion and caring heart just poured out after she heard my niece’s story. Even though their office won’t be able to help with what my niece needs, she still went above and beyond to offer help. She took initiative to call the surgeon’s office to get the surgery info to help moving forward. In today’s world, you don’t see or hear something like this happen anymore, I am very grateful for experiencing this in Davis, CA. It gives me hope for the society and the values people still hold. They can do this for a stranger, can you imagine what they will do for their own patients ? So if you are looking for an oral surgeon’s office, I highly recommend the Center to you! They don’t know that I’m posting this ( I normally don’t), this is from the bottom of my heart. Thank you Angela, I wish the best for you and the Center. Wen DePriest

I don’t always share on here but I wanted to. I was terrified to get my wisdom teeth taken out. I have been avoiding it for years and finally I broke one of my wisdom teeth was having such bad pain from the break I had no choice. I searched for a few days for an oral surgeon to take them out and all had a long wait time. I called on a Friday and by Monday I was getting my wisdom teeth removed. Mind you this was days before Christmas. So I was so glad to find a place to squeeze me in on short notice. Even though I was terrified, the pain was so bad I needed to do it. The team was so friendly and supportive even though I was extremely afraid. They talked me through the prep process and that was super helpful. I wish I could remember the woman’s name who held my hand while I was going under she was truly so helpful. I was so nervous and afraid and she was very friendly (and so was everyone else) but her holding my hand really helped me when I was scared. It’s been 10 days post op and I’ve healed up very well. I still have some soreness and discomfort but every day it gets better and I’m sure I’ll be back to myself in no time. I highly recommend this place if you need oral surgery.

My daughter had to have all four wisdom teeth removed, a week before she moved into her college dorm at Davis. She was so nervous and had to be fully put under. It was extremely hard for me waiting in the waiting room and not being there for her, but, from start to finish everyone was absolutely amazing. The gals at the front desk kept me distracted while the surgeon and his team took great care of my baby girl. Let me just add, we were supposed to have her teeth pulled at another oral surgeon, but something about them made me so uneasy and overly anxious. I didn’t think I had an option to take my girl somewhere else since they were referred by our dentist. However, motherly instinct set in and I called Davis Oral Surgery, they verified insurance, did a consult and scheduled her. All within a week. Thank you Angie and your partner (shame on me for not knowing her name), for your kindness and PATIENCE!! even though I called like five times. I was so stressed and overwhelmed and from the moment I called you, and you assured me it would be okay and I felt so much better. It doesn’t seem like a big deal to most, but my daughter is all I have so everything is a big deal to me. So thank you! Thank you 🙏🏼

Such a friendly office that shows they care about the patient. It was very comforting to have skilled professionals pull my husband's tooth. They made him feel relaxed and pulled hi bad tooth in a short amount of time. Thank you so much for the wonderful care you took with him.
